Procedures

Licensure as a Licensed Marital and Family Therapist (LMFT) in Iowa is governed by the Iowa Board of Behavioral Science, now administratively housed in the Department of Inspections, Appeals, and Licensing (DIAL). The “permanent” LMFT is the full license that follows a period of supervised clinical experience (often under a temporary LMFT license).

Below is a focused guide to the current requirements, emphasizing the exact categories of hours and how the Board describes them.

Overview of the Iowa LMFT Permanent License

To obtain a permanent LMFT license in Iowa, you must document all three of the following:

  1. Educational requirement – qualifying master’s or doctoral degree in marital and family therapy (or a content‑equivalent field). (law.cornell.edu)
  2. Examination requirement – passing the national AMFTRB Examination in Marital and Family Therapy. (dial.iowa.gov)
  3. Training/supervision requirement – either:
    • Proof of clinical membership in the American Association for Marriage and Family Therapy (AAMFT), or
    • A completed Supervised Clinical Experience: Supervision Report documenting your supervised clinical experience hours. (dial.iowa.gov)

The supervised clinical experience hours are the core of what distinguishes the temporary license from the permanent LMFT.

Educational Requirements

The Board’s rules describe two main pathways for education:

1. COAMFTE‑accredited MFT program

You must submit an official transcript showing completion of:

  • A master’s degree of 60 semester hours (or 80 quarter hours) or a doctoral degree in marital and family therapy
  • From a program accredited by the Commission on Accreditation for Marriage and Family Therapy Education (COAMFTE)
  • At a college or university accredited by an agency recognized by the U.S. Department of Education. (law.cornell.edu)

2. “Content‑equivalent” non‑COAMFTE program

If your program is not COAMFTE‑accredited, you must show:

  • A master’s (60 semester hours) or doctoral degree in marital and family therapy, behavioral science, or a counseling‑related field, from an accredited institution, and
  • That the degree is “content‑equivalent to a graduate degree in marital and family therapy.” (law.cornell.edu)

For non‑COAMFTE programs, the Board requires an equivalency evaluation by the Center for Credentialing and Education (CCE). This CCE content‑equivalency report must be sent directly to the Board and is in addition to your official transcript. (dial.iowa.gov)

Examination Requirement

Iowa requires passage of the:

  • Association of Marital and Family Therapy Regulatory Boards (AMFTRB) Examination in Marital and Family Therapy.

The DIAL instructions list as an application attachment: “Proof of passing the Association of Marital and Family Therapy Regulatory Board (AMFTRB) Examination in Marital and Family Therapy.” (dial.iowa.gov)

You typically take and pass this examination during or before your supervised practice period so that, by the time you apply for the permanent license, the Board already has your passing score.

Supervised Clinical Experience Requirement

(Post‑degree hours that qualify you for the permanent LMFT)

Iowa’s supervised hours are governed by Iowa Admin. Code 645–31.7, “Supervised clinical experience”, which applies to both mental health counselors and marital and family therapists. (law.cornell.edu)

The rule uses specific terms for different categories of hours:

Board’s Key Hour Categories (Current Rule)

From 645–31.7(1), the supervised clinical experience must meet all of the following minimums:

  • “Minimum number of hours” – 3,000 hours of practice

    • The rule states that “the supervised clinical experience must consist of at least 3,000 hours of practice.” (law.cornell.edu)
    • This 3,000 figure is your total supervised clinical practice (direct + indirect activities).
  • “Minimum number of direct client hours” – 1,500 hours

    • The rule specifies “at least 1,500 hours of direct client contact.” (law.cornell.edu)
    • This is what many people informally call direct experience: actual therapy or clinical services with clients (individuals, couples, and families).
  • “Minimum number of direct supervision hours” – 110 hours

    • The rule states that “the supervised clinical experience will consist of at least 110 hours of direct supervision” that are equitably distributed across the supervised period. (law.cornell.edu)
    • Up to 50 hours of these 110 hours may be obtained through group supervision.
    • Supervision may occur in person or via videoconferencing.
    • After the initial 110 hours, ongoing direct supervision must continue for the remainder of the supervised clinical experience. (law.cornell.edu)
  • Duration – minimum two years

    • The supervised clinical experience “must be for a minimum of two years.” This is a time requirement in addition to the hour counts. (law.cornell.edu)
  • Timing – must be post‑graduate

    • The rule states that the supervised clinical experience “cannot begin until after all graduate coursework has been completed with the exception of the thesis.” Practicum and internship hours from the degree do not count toward this 3,000‑hour requirement. (law.cornell.edu)
  • Number of supervisors – up to four at once

    • A supervisee may “utilize a maximum of four supervisors at any given time” and must notify each supervisor if others are being used so supervision can be coordinated. (law.cornell.edu)
  • Eligible supervisors
    A supervisor must:

    • Hold an active license as an independent level social worker, mental health counselor, or marital and family therapist in Iowa;
    • Have a minimum of three years of independent practice;
    • Have completed at least a six‑hour continuing education course in supervision or one graduate‑level course in supervision; and
    • Be knowledgeable about the applicable ethical code and licensing rules. (law.cornell.edu)
  • Content of the supervised experience
The rule requires that supervised clinical experience include:

    • Performing psychosocial assessments
    • Diagnostic practice using the current DSM
    • Providing treatment, establishing treatment goals, using evidence‑based modalities, and engaging in differential treatment planning
    • Training in practice management, ethical standards, legal and regulatory requirements, documentation, coordination of care, and self‑care. (law.cornell.edu)

How the Hours Break Down in Practice

Putting the Board’s wording into plain language:

  • 3,000 hours of “practice”

    • This is your total supervised clinical work, completed after your graduate coursework is done (excluding the thesis).
    • It includes both direct client time and related clinical tasks (documentation, team consultation, treatment planning, case review, etc.).
  • Within those 3,000 hours, at least 1,500 must be “direct client contact”

    • These hours are face‑to‑face (or telehealth) clinical services delivered to clients (individuals, couples, or families).
  • Within the overall experience, at least 110 hours must be “direct supervision”

    • Regularly scheduled meetings with an approved supervisor or supervisors, in person or via videoconference, including discussion of cases, review of clinical work, and ongoing evaluation of your development as a therapist.

Earlier Iowa materials and some secondary resources still mention 3,000 hours plus 200 hours of supervision; those reflected older rules. Current Board rules, as updated through the Iowa Administrative Bulletin and codified in 645–31.7 and 481–880.7, now require 3,000 total practice hours, 1,500 direct client contact hours, and 110 supervision hours. (rules.iowa.gov)

How These Hours Connect to the “Permanent” LMFT License

Temporary vs. Permanent License

The AAMFT’s Iowa state summary and Board materials describe two levels of MFT licensure:

  • Temporary Licensed Marital and Family Therapist – allows you to practice under supervision while you accumulate your 3,000 hours of supervised clinical experience.
  • Licensed Marital and Family Therapist (LMFT) – the “permanent” license – granted after you complete education, pass the exam, and finish the supervised clinical experience as defined above. (aamft.org)

Documentation Options: AAMFT Clinical Membership vs. Supervision Report

When you apply for the permanent LMFT license, DIAL instructs you to attach, under “Training/Supervision Requirement,” one of the following: (dial.iowa.gov)

  1. Proof of clinical membership in the American Association for Marriage and Family Therapy (AAMFT)

    • Clinical membership in AAMFT entails meeting national standards for education and supervised experience. Iowa’s Board accepts this as proof that you have met the training/supervision requirement.
  2. Completed “Supervised Clinical Experience: Supervision Report” form

    • If you do not have AAMFT clinical membership, your Iowa‑approved supervisor(s) complete this Board form to verify that you have met the supervised clinical experience rule (3,000 hours practice, 1,500 direct client hours, and 110 supervision hours over at least two years). (law.cornell.edu)

In either case, the underlying standard is the supervised clinical experience rule; the Board simply offers two acceptable ways to document that you have met it.

Step‑by‑Step Summary of the Path to Permanent LMFT in Iowa

  1. Complete a qualifying graduate degree

    • COAMFTE‑accredited MFT degree (60+ semester hours), or
    • Content‑equivalent master’s/doctoral degree with CCE evaluation if non‑COAMFTE. (law.cornell.edu)
  2. Obtain a temporary LMFT (or equivalent) and begin supervised practice

    • Develop and submit a supervision plan and work under a Board‑approved supervisor (or supervisors). (law.cornell.edu)
  3. Accrue supervised clinical experience after coursework completion

    • At least 3,000 hours of supervised practice, including
    • At least 1,500 hours of direct client contact, and
    • At least 110 hours of direct supervision, distributed over a minimum of two years. (law.cornell.edu)
  4. Pass the AMFTRB Marriage and Family Therapy exam

    • Ensure the Board receives official proof of your passing score. (dial.iowa.gov)
  5. Apply for the Permanent LMFT License

    • Submit online application and fee (currently $120).
    • Have your official transcripts sent directly to the Board.
    • Attach either:
      • Proof of AAMFT clinical membership, or
      • The Board’s Supervised Clinical Experience: Supervision Report completed by your supervisor(s). (dial.iowa.gov)

Once the Board verifies that your education, supervised clinical experience, and exam meet Iowa’s requirements, it issues the permanent LMFT license.
